The integration of health and social care has been a policy ambition for many years. The HDCP Information Day brings together key parties in order to break down the barriers and challenges to achieving integration. Together we must focus on better outcomes for individuals and design our systems and services to invest more to prevent or intervene early before health and care needs escalate.
Be there on the 29th March to have a say in helping shape health and social care in Hampshire.
